Eligibility
All registered full-time students as well as degree-seeking co-op students are eligible to enroll in Purdue’s student insurance.
Domestic students are not required to take the student insurance, but you are expected to have coverage outside of Purdue. Online only students are not eligible for student insurance.
All international students who are registered for classes, regardless of credit hours, visa status, or insurance policy type, are required to enroll in the University-sponsored medical insurance plan or to obtain an approved waiver of coverage by the enrollment/waiver deadline.

Vision Claim Form (out-of-network providers only): To download a claim form, go to www.anthem.com and log in or click Individual & Family, then click Forms under the "Support" column. Choose your state, and then scroll down to Claims and select the Blue View Vision Out-of-Network Claim Form. You may instead call member services at 1-866-723-0515 to request a claim form.
Fellowship Vision Enrollment Form (for "True" or "Traditional" fellows)
Purdue University requires all international students, regardless of visa status, insurance policy type, or number of credit hours taken, to participate in a University-sponsored medical insurance plan or to obtain an approved waiver of coverage.
The Spring 21-22 waiver deadline ended 02/01/2022 at 5PM. The online waiver system can no longer accept new waiver requests.
